Use this card when you need to understand or check an AI-generated answer.

1. Given

• What did the user provide?
• What instructions were supplied?
• Were any files, documents, or tool results included?

2. Generated

• The available text became tokens.
• The model chose a likely next token.
• The token was added and the process repeated.
• I remember that fluent wording is not proof of truth.

3. Added abilities

Which parts are proven?

• Search or retrieval.
• Calculator or code tool.
• Saved memory.
• Citations linked to found material.
• Agent steps or outside actions.
• Human approval.
• None of these are proven.

4. Support

Mark each important claim:

• Given: stated in the input.
• Supported: backed by a suitable source or tool result.
• Added: introduced by the model but not yet supported.
• False: checked and found to be wrong.
• Missing: an important point was left out.

5. Next step

• Keep supported content.
• Remove invented content.
• Correct false content.
• Add missing content only after checking it.
• Check citations outside the generated answer.
• Ask an authorised person before an important action.
