What this module established
Place the gate immediately before the first irreversible step - anywhere later is a notification wearing a control's interface. Show what will happen, the evidence, what was not checked, and what happens if the reviewer does nothing.
Carry forward
- Structure rejections into what was wrong, the correct value, and the next action. A free-text box produces 'not right', which nothing can act on.
- Track edit rate and seconds on screen. Both falling towards zero means approvals have become reflexive, and the fix is fewer gates.
- Record what evidence was shown. An approval on incomplete evidence is a system gap; one on complete evidence is a judgement call.
Before moving on
Move on when your gate sits before the first irreversible step, rejections change the retry, and you have a baseline for edit rate and reading time.
