Turning notes into actions and owners

The main idea
Action lists need clarity on task, owner, due date, and open question.
An action missing any one of those will not happen, and the most commonly missing one is the owner.
Why this matters
Generated action lists tend to be complete and unassigned, which feels productive and changes nothing.
Safe example
A messy note becomes owner, task, due date if known, and uncertainty.
The uncertainty column is where the honest gaps go instead of being quietly filled.
Better working habit
Use placeholders when owner or date is missing.
"OWNER TBC" is a visible gap and a prompt for a conversation. A plausible name is neither.
What to watch for
Do not invent owners or deadlines.
A name against an action nobody agreed to is worse than a blank, because nobody will query it.
