The main idea
The final product should be useful and reviewable.
Useful means a reader knows what to do. Reviewable means someone who was in the room can check it against what happened.
Why this matters
A follow-up note becomes the record. Anything it states confidently will be treated as agreed, including the parts that were not.

Safe sample notes become summary, actions, questions, and next step.

Better working habit
Keep unclear items visible instead of hiding them.
An "open questions" section is what stops uncertainty being smoothed into the summary above it.
What to watch for
Do not make a follow-up note sound more certain than the notes support.
Confidence in a record is not neutral. It removes the reader's reason to check.
