The main idea
The output should help a real reader, not only look organised.
Organisation is easy to produce and easy to mistake for usefulness. A tidy document that does not answer the reader's question is still a failure.
Why this matters
The check is what the reader does next. If the outline does not lead somewhere, no amount of structure will rescue it.

A short document outline includes reader, purpose, sections, evidence needs, and review notes.

Better working habit
Save the prompt brief and the reviewed outline.
Both, not just the outline. The brief is what you reuse next time; the outline is specific to this document.
What to watch for
Do not include confidential source notes.
Outlines get shared more widely than drafts, because they look preliminary. Anything in one should be safe to circulate.
